AUCA • EDUCATIONAL WORKSHEET — NOT AN OFFICIAL AGENCY FORM
Pet incident documentation worksheet
Seek veterinary care first. Complete this only when safe.
What this is for: Use this optional worksheet to organize your own notes about a pet incident, such as what you personally saw, when it happened, veterinary care, and any agency contact. You can keep it for your records or share it with a veterinarian or appropriate authority if helpful. It is not an official agency incident report; completing or printing it does not submit anything to AUCA or request an investigation.

Important limitations
Do not pursue wildlife to collect evidence. Keep private addresses and personal details out of public posts.
For immediate danger to people call 911. For wildlife guidance consult the Arizona Game and Fish Department or an appropriate local authority. AUCA does not dispatch responders or investigate incidents.
